Overview:
In the past 12 months alone, over 43.5 million caregivers have provided unpaid care to an adult or child. There is abundant evidence that many family caregivers are not given the support they need to provide this care – it can be quite substantial in scope and duration. Often, there is a huge gap between hospital discharge and care for the patient in the home. Helping professionals play a crucial role in identifying and helping to bridge those gaps to ensure quality continuity of care. In this thought-providing presentation, experienced social worker and discharge planner, Jill Powell, MSW, will provide a comprehensive overview of what caregiving in America looks like – the
weight of responsibility, burnout, the need for respite and support, and helpful services available in the community. Participants will gain the information and resources necessary to support caregivers and patients requiring additional support in their healing and wellness at home.
